If you are new to the area, and would like to register with us, please attend the surgery during opening hours and our receptionists will give you the appropriate paperwork to complete in order for us to register you and request your medical records from your previous GP. It is helpful if you can complete our New Patient Questionnaire in the interim to us receiving your medical records, so that we have details of any repeat medication that you are taking and any major illnesses or operations that you have had in the past. You will be provided with a practice brochure to assist you with how the surgery operates and may be offered an initial health-check appointment with our Healthcare Assistant.
Registration documents should be comleted and returned to our reception team.
