COPD Review

We want you to stay on top of your COPD, to minimise the risk of it getting worse or flaring up.

National guidelines recommend a COPD review once a year, and we aim to carry these out in/around your birthday month. You may also be asked to complete a COPD review at another time.

During the Covid pandemic, our nursing team are carrying out most reviews remotely, through a combination of online and telephone consultations. You will first complete an online form using the link below, and this information will then be reviewed by one of our COPD nurses. Once you have submitted your information online, you will be contacted to arrange a telephone appointment to speak to one of our COPD nurses. Please include any questions or concerns in your online form, so we know to discuss these with you when we call.

How long will it take me to complete the online COPD review form?

It should take no more than 5-10 minutes.

Why am I not being offered a face-to-face review?

Because of the Covid pandemic, we are trying to minimise the number of people in our surgery building. Most people can have their annual reviews through a combination of online and telephone consultation, so a face-to-face appointment is not usually necessary. Doing things this way saves patients from having to visit the surgery, reducing risks to our patients and staff, and reducing our use of scarce PPE. This means we can maintain a safe face-to-face service for patients who need to see us for an examination.

Who will review the information on the form I submit?

As soon as your form is received in the surgery, a member of our patient services team will allocate it to one of our COPD nurses. The nurse will then review your information in detail, in conjunction with your medical records, and check whether any further action is needed. If necessary, they may arrange for you to speak to them on the telephone. The nurse will also update your COPD medication if appropriate.

How do I request more medication?

If you need a further prescription of any of your regular medication, please use our prescriptions page.

What if I need my repeat prescription updating?

As part of the COPD review, the nurse will review and update your medication accordingly. This is usually updated for 12 months, and will be reviewed again at the next annual review.

My COPD symptoms are causing me problems. Can I review my COPD more frequently than every year?

If your COPD is causing you new problems, you should contact us online to let us know straight away using eConsult. We will review this information by the end of the next working day and arrange a telephone or face-to-face review with a doctor or prescribing nurse as appropriate. You should not wait until your annual review to tell us about any worsening of your COPD.

Equally, if you are having a problem with your medication, or have another query or concern you can contact us in the same way.

In addition, we may recommend that we review your COPD symptoms more frequently, for example because of a recent exacerbation or changes to your medication.

Please note that you should telephone 999 if you have severe difficulty breathing, or if you are gasping for breath or struggling to get words out, choking, or your lips are turning blue.

What will my COPD review involve?

Your annual check will usually include:

Asking you how you feel your COPD is at present and how it affects you
Assisting you in getting to know what is normal for you and when it worsens what to do
Discussion around how to manage exacerbations (flare- ups)
Discussion around inhaler management and a check that you are using them correctly
If you still smoke, you should be offered support to stop
Advice around keeping active and eating well
Discussion about the benefits of Pulmonary rehabilitation and a referral offered if appropriate
A written care plan will be agreed and given to you (COVID-19 restrictions means we are conducting these checks over the phone so it may not always be a written plan at present)
You will be offered the Pneumonia vaccine if you haven’t already had it
You will be offered an annual flu vaccination
